no queen but an open queen cell

In one of my hives there is no queen and there is one open queen cell.
This hive is very strong and there are larvae.
Does anybody know what has happened?

Squished between two end bars?  I've seen them where I squished them when I put the lid on, where they got stuck in the queen excluder, where I removed them along with a feeder...
